Yep, I am 1500 miles from home, but I can tell you that is a Hynautic K-6 cylinder.
Mike Charman of Charman Enterprises is (was) the factory parts distributor before Morse sold out to Teleflex since I retired. Teleflex sort of dropped everything. www.charment.com

I really don't know if it has instructions or not. I retired from Hynautic in 1999. Teleflex may have made some changes in the kits since then. When I was there it just had a blown-up parts picture.
