If the driver punches it out of the hole, the skier will mostly likely dislocate an arm or 2 from the shoulder. Use a progressive, semi aggressive acceleration from the start. It is easier to wait out a slow start than to hang on to a too-quick start.

Depends on the boat. Behind my 3.0L, there is no danger of getting your arms ripped out of the socket - just drowning before it pulls you out of the water ;)

Agreed, every boat will be different. An 18' direct-drive ski boat with a 350HP V8 might rip the rope right out of your hands, but a 90HP 2-stroke outboard on that same 18' probably won't make you stress too much.
