I recently bought this outboard and started it for the first time yesterday. Runs like crap and air comes out of the little broken hose. Whats wrong here, should it be connected somewhere?

there is nothing on a Yamaha 2 stroke 90 that normally pumps air out of a small hose

Were you running the motor on a hose ?---Boat launched and at a dock ?

the only small hose coming off of an outboard that I can think of would be for a speedometer.
but I have no idea how it could be blowing air out when motor is running on muffs

Here is the whole head with the hose plugged.
I noticed that the colour of the cylinder part don't match head and intake. Someone might have replaced it back in time.
There are two hoses going between intake, and one from intake to cylinder 2, should it be four hoses in total?

This motor has been apart for some reason.----All bets are off as to the reason for that hose.-----It leads to the crankcase and I see no reason to tap into crankcase compression !

recirculation of liquid condensed fuel from the bottom of each cylinder's crankcase is the only thing I can think of.
should be check valves involved at least one end
