You won't need a module for your Ford truck so don't worry about that.

Only if you want a hack job.

If the job is done correctly, with proper testing and solid, secure and protected connections, expect up to 1 hour. Or the shop may use a "T" connector which would cost more, but is a better (in my professional opinion) installation of a flat four connection.

You should check the fuses under the hood.

DANG you beat to this possibility. I love it when a customer comes in with the complaint of turn signals, brake lights or running lights out and they've checked all the fuses. When I check the fuses, replace one and show the customer that the lights now work, they look at me like I'm a genius. I'm purty schmart, but I'm not no genius (read with redneck accent:)).

Make sure to consult your owners manual. Since all your lights work properly with another truck, I wouldn't worry what the 5th wire is for. The problem is evidently with your truck.
