Is there any way in which something can happen to the crankshaft inside the crankcase that would make it no longer line up where it should when the Piston is at top dead center? I'm wondering if something inside has gone wrong. Also what does the crankshaft connect to that holds it in the proper position? Is it connected to the drive shaft

The little dimple on the camshaft gear (big gear) should align with a corresponding dimple on the crankshaft gear small gear).
