2008 Signature 310 trim tab actuator died

One of my Lenco trim tab actuators stopped working and as luck would have it, I just put it back in the water 2 weeks ago. Does anyone know what the model number is or has theirs out the water and can send a photo of the numbers or what the part numbers are.

Please post part number only for Signature 310 year models 2004 - 2009 only. I'm concerned the later models may be different.

Per Lenco's web site its stamped on the largest barrel (see photo below for location). Chaparral parts look up doesn't list the model.

If anyone was having the same issue, I contacted Lenco and they said actuator 15057-001 is the part number IF your Chaparral Signature 310 parts guide kit number is 10021-001. Snipping from Chaparral parts guide below.
