I would definitely change the fuel pump diaphragm kit as ethanol is hard on those old rubber parts. If ethanol hasn't torn it up, then 15years of age took it's toll. Just change it, you will be glad you did. The biggest pain is getting the hoses on/off the pump, the rest of it is a snap. Remove 4-6 screws, replace a couple gaskets and rubber parts. Probably took me 1/2 hour,,, 15 minutes of that was cussing the hoses on/off.
But, from your description, it sounds like you might be sucking air into the fuel line. While the OB is experiencing the problem (running WOT), squeeze the primer and see if the outboard responds. If squeezing the primer bulb makes it run better, start looking at the fuel hose, fittings and tank, look for a fual/air leak. (If you need a new fuel line quick disconnect for the OB, don't get a "Mallory", mine only lasted for a couple months, then started leaking again, poor design.)
If that's not it, I'd look at rebuilding the carb as suggested earlier.
