1987 5.0L OMC Cobra shift lever came out of shift guide Why?

My 87 bayliner 5.0 stuck in reverse wouldn't go to neutral or forward took off transom nothing seems to be broke or in bad condition just the shift lever was out of the u shape of the shift lever guide, wondering what might cause this?

Also when put the lever back into guide and moving by hand the roller on end of lever just is sitting in top of slot of guide in neutal postion is this right? seems it wouldn't take much for it to pop out again.
