Volvo Penta 5.0FL Runs Great With Garden Hose & Won't Start in Water

I have a Volvo Penta 5.0FL that will run perfect with the Garden Hose attached, but when i put it in the water it won't start up with out throttle high. Whats going on? Is it the Carbs?

Yep, I think it is running lean.
