I believe the gear your talking about drives the distributor (via a shaft). Whoever installed the belt last probably just moved the plug wires around to compensate,or twisted the he!! out of the distributor.

If you are going to pull the belt, make sure you have the Volvo service manual (Not aftermarket) for your engine.
It explains how to reset the distributor correctly. Be prepared with a timing light to retime the engine.

Depending on the ratio (size of the 3 gears or cogs), that distrib drive cog may need to spin 70 or more times to rotate back around to where the dot will line up again. Do as Don says and follow the manual. It is not that difficult, but pay attention to detail....
