how best to do the manifold riser joint on a 3.0LGS

Hi, this has been an ongoing problem for as long as i can remember with the boat. If ever the boat starts to vibrate/run rough on idle/only run on 3 cylinders I now go straight to the riser/manifold. two years ago i replaced riser and manifold. Last year it turned out manifold was faulty/leaking and it was replaced under warranty. I now have the same issue.
On the muffs it will idle for ever and a day very smoothly, out in the sea idle idle smooth for 1 or 2 mins then drop a cylinder and run rough. A little rev out of gear clears the roughness. It also does it if we are running at low speed for a length of time, it then bogs down before pulling away, unless we rev it out of gear before pulling away.
Why would this be happening at sea, even in flat conditions but perfectly fine on the drive.
I removed the riser and yup water in manifold again! I have heard of a manifold extension to raise the riser away from the sea. Would this help? But i cannot see how the rubber boot then joins the exhaust to the riser.
Also my boat never had the restrictor plate, I fitted it once and found it leaked even worse so ditched it and went back to one gasket/one joint.
Honestly I think this design of manifold/riser is the worst that could very have been thought up. Both riser and manifold surfaces look better than good what would you guys do, do you think water is coming back down from the sea side , we tested for a blockage in the exhaust, i really don't think there is one, water poured out of both the prop and when we moved the hose back a bit in side the exhaust to came out the exhaust before the leg ( im not sure what that is called)

does your boat have a flapper at the top of the exhaust pipe that connects from the exhaust elbow to the transom mount?
does this boat sit particularly low at the stern?

Hi no the exhaust flapper has never been fitted since I had it. 15 years now . It’s a similar boat to yours 99 fourwins 180 horizon . I would say the water line is roughly a foot lower that the bottom of the riser when at rest.

Is this an OMC or Volvo power package? OMC's only went to '98, there was a joint venture between them from '94-98 and after that Volvo bought out the remainder of OMCs sterndrive operation, and continued production, but the engines/drives were pretty much identical from '94-98 and your 99 boat could have a 98 power package. In the parts diagrams I've looked at I do see a flapper if it was a '98 OMC package, I know Volvo discontinued using the flappers at some point (right around '99) but not sure what year yours is. Model number helps to determine what it is.

The usual # seen for the height of the riser above the static water line is 12-13", so it may not be too low....post up some pix...

serial number is 4011312544 engine model is 302C PBYC thanks
however we have put a new GM engine in 3 Years ago and used all the existing parts/carb/ect ect. Looks just like a Volvo Penta to me. The hand book is Volvo Penta with an OMC COVER 🤷‍♂️

My problem seems to be whilst at idle or very low speed . At that point I’d need the flapper to be open any how, not shutting back then I would think even more water would dump into the last cylinder.
I have tried the restrictior plate once in the past. It seemed to make the joint even worse. So I removed it after a couple of outings.
what’s people’s thoughts on the restrictor plate, are you still fitting it a mechanic told me not to , as was the advice on the flapper
