I have a 2000 150hp Johnson. I have been using the flushing port (not using ears) and connecting the hose up to that to flush it. HOwever, I have been running the engine during flushing. Has this damaged the water pump? How concerned should I be?

most likely, no water impeller gets hot. flush port is for motors that are more often permanently in the water.

If it is still peeing fine, I should still take the water pump out and check it?

Timeking, Using the flushing port and running the motor is perfectly fine, according to the owners manual for that motor. The flushport provides water to the impeller as well.

Well I replaced the pump. There was no evidence of any damage to the impeller or the cup.
