Can anyone identify this?

Any ideas who might of made this controller? We are looking for the cover that goes over the wires, any ideas where one might find a replacement?

Try making a pattern and cutting one out of mahogany ply, varnish ansd use a bit of silicone to hold it in place. I bet it would like nice.
