2006 200 Optimax problem

Motor has 152 hours on it and Cylinder #2 blew apart - cylinder #1 showed signs of wear also and was ready to go. Anybody else have similar problems.
Motor codes come out clean showing no problems.
Water ?Pressure fine - no alarms - was just running at 500 RPM and she shut down with a hole in the side of the block.

possibly a faulty warning sensor didn,t go off. lack of oil overheated and bang!!!! all done. not firmiliar with your engine

Sorry was a couple of typos in original post - 5000 RPM's I had just got up on plane and went about 1/4 mile - water pressure was fine. Injectors checked out ok. Replacing cylinder #1 & #2 injectors JIC.
