I would just take a hub off of it and take it down to a Napa or similar parts store in your area and they can figure out what you need. Many times identifying the trailer does not lead to getting the correct parts. Trailer manufactures use all different types of companies when they are building trailers. I have seen trailers from a manufacture, same exact trailer with these different companies axles on them, they used what they could get for the best price and changed mid stream production to keep the line going.

I would make sure and go a head and get new bearings and seals as well, then you have nothing to worry about for a while into the future.

12 inch tires would imply it is a 2000# axle and has1-1/16 inner and outer bearings. As mentioned, pull the hub, clean the bearings and races and jot down the numbers. They are very generic and available most anywhere boating supplies are sold including NAPA and most other auto stores. Then there is also right here on IBOATS.
