1971 Mercury 500 50 horse. Runs with muffs and water hose, no run in water

talking to a guy local about a boat motor. It is a 1971 Mercury 50 Horse motor and will crank and run when hooked up to a waterhose with muffs, but will not start when put in water. Doesnt make any sense to me. Any opinions? Cures>?

More backpressure when the exhaust is submerged.

Do a compression check on that motor.
