Have you done basic checks for compression and spark? A motor can run wonderfully on muffs and not in the water due to backpressure on the exhaust system, and having to actually pump water through the cooling passages rather than relying on the hose to force water into the system. A prime culprit would be a cylinder not running. Check for proper compression and spark on all cylinders.
