The first problem is that it is not running good in Neutral; it is faking it, which is easy with no load.
Most commonly it has a dead cylinder. Next is slipped linkage that is not applying throttle.
Check for a dead cylinder by disabling each cylinder in turn (pull spark plug wire). Simple inspection should find bad linkage.
