1988 70 idle problems

i have a 1988 e70elcca S/N g1800349. I have done a rebuild on the carbs and it still doesnt idle good sometimes. I have the factory service manual and to do the link and sync i use method B. i did the throttle valve sync which included moving the throttle cam follower away form the cam, Then i did the idle speed adjustment, then the throttle cam follower adjustment. What I dont understand is how can the idle be set first then when doing the throttle cam adj you have to have .005 clearance between the cam and follower. HOW CAN IT IDLE IF THERE IS CLEARANCE BETWEEN THE LINKAGE?? I hope i am just missing something here

the throttle valves stay shut at idle -- idle speed adjustment is all spark timing.

thanks. now when i do the throttle cam follower adjustment it says arm at idle do the mean in gear or out of gear?
