The water leaves with the exhast, but you'd expect some droplets spray or mist from the relief hole on the leg as well.
You should be able to touch the cylinder head for a second or two but if it is too hot to touch or sizzles if water is splashed on it then it is overheating and probably needs a new pump impeller.
