Using straight hose as replacement tell-tale - 1993 Mercury 60HP 2 stroke 3 cylinder
The tell-tale on my 1993 Mercury 60HP 3 cylinder 2-stroke twin-carb outboard had a rip in it, so it was spraying water under the engine cover. I have removed it and put a longer hose on that pokes out through the hole. I have tested this with bunny ears and the water flows out no worries.
Is it fine to use like this? I am just concerned that the original tell tale only has such a small hole for a specific reason - e.g. to create back pressure in the cooling system?

I think yer right. Most of the water should go into the mid to cool the exhaust. The little plastic thingie should pull off the old hose and insert in the end of the new one.

My concern is not in relation to the hose length, I see no reason why that would effect anything. My concern is in relation to the diameter of the exit hole. If you look at the exit hole on the tell tales it's usually about 2mm ish, since I am just running straight hose and not the telltale, it has a larger exit hole (about 6mm ish) - this may cause the cooling system not to build up some pressure at the top which would encourage water to exit out the mid engine instead, or it may do nothing, I'm not sure.

No worries about that setup you have. The telltale is located at the "end" of the cooling system, after it has passed thru the motor. It is only there to give an indication that water is circulating thru the motor. You are not robbing water from reaching the powerhead by increasing the hose outlet size.
