Having a hard time figuring out when you would use an adapter like this.

Couple of things come to mind;

Towing a dual axle trailer.
Towing tandem trailers.
Towing a double-wide
Towing a tri-hull

Obviously - I have no idea, Roscoe. Where did you find it?

They used to sell a lot of those in my area, where it is legal to double tow, I can tow my 5th wheel and my boat at the same time, you would hook your main trailer into one side and run a second pig tail back to the boat, now a days, most of us just splice into the trailer harness at the back., if I remember right, there is a balancing resistor in that black barrel where the wires split.

found it at cabala by accident.

I haven't been around many 5th wheel trailers, but don't recall any of them using a flat 4 plug.

There are a number of supplemental lighting situations that you could use this plug on, most 5th wheels these days have 6 or 7 prong connections, but there was a day, when 4 prong for lights was all you had.
