Yamaha 250HP 2 stroke, burnt piston, #6

Hi,
New to the site, working on a 2003 Yamaha 250HP 2 stroke outboard. Pulled one of the heads and found that #6 piston is burnt and parts of it miss near the cylinder port. I have been looking around the web and it seems this maybe a common problem. Can anyone share with me if there is some defect in this engine? Thanks

No defect, just leaned out a cylinder. Probably due to dirty fuel system.
