Grinding sound while shifting from neutral to forward or reverse (91 Yamaha 5.0)

I have a 1991 Yamaha stern drive. Among the many issues I'm having with it, it grinds when entering either forward or reverse. The boat has done this so long as I have owned it (over 10 years) and hasn't been an issue necessarily, but it doesn't seem like something that is normal.

What would cause this type of thing?

How are you shifting? A good quick movement out of neutral into gear before RPMs increase is usually the best way. Granny shifts almost guarantee grinding.

What does the gear lube look like when you change it? Any metal grinding dust in the lube?
