I just bought an old boat, good shape, unused for 2-3 years. It has a built-in fuel tank with no access, full of stale gas and unknown water and sludge. After siphoning it out, how can I clean it ? Is a good filter / water separator sufficient to protect the engine (outboard) ?

Just my thoughts;
It shouldn't have sludge because gas is pretty clean. The little water that may have been in there came out with your siphon job. Can you see in the tank at all to see if it is rusty?
I would not get too crazy, I would by a good spin on water seperator and I put and additional Fram type inline filter right at my engine, put 10 gallons of gas in there and run it easy through that tank. Replace the filters and I would think you are set.
