I believe A is supposed to attach to the threaded end (not visible) of the steering tube C. I've been using violence to force D into C but that approach has suddenly ceased to work. Have applied heat to C, no change. How is this supposed to work?

remove (D) and clean out bore of (C) with a stiff round wire brush And penetrating oils. BE AWARE!! when using heat that carb and fuel are above (C) Are you sure you have the right cable (C) ??

Yep. Clean out everything. Lube everything. If it still won't go, get a new tube and a new cable. In the future, keep that area well lubricated.
